Important Things to Know for 2011-2012

  1. The Liberty-Eylau ISD Cafeteria Plan year runs from September 1 through August 31 of each year. All changes and new products for this plan year will be effective September 1, 2011


  2. NBS will serve at the new Third Party Administrator (TPA) for the medical and dependent care reimbursement accounts for the plan year September 1, 2011 through August 31, 2012. Also new this year will be the inclusion of a Debit Card that you have the option to use – There will be a $1.50 tax-free monthly fee for the use of the card if you choose it. Receipts must be maintained in the event of an audit. See brochure for more information.


  3. Loyal American Cancer: The cancer plan is offered again as a supplemental insurance. Any employee who wishes to have this coverage must complete a paper application and be approved for coverage. Coverage can be taken with or without the Intensive Care Rider.


  4. Unum Disability is offered again to all employees on a guaranteed issue basis. Employees have two plan options to choose from; one plan pays benefits all the way to age 65 and the other pays to age 65 for accident but 5 years for sickness disabilities. Guaranteed Issue means all employees may enroll without having to complete a medical questionnaire. **New coverage or changes/increases in coverage are subject to a 12-month pre-existing condition exclusion.   5. 5 Star Group Term Life: All new coverage or increases in coverage are subject to evidence of insurability. An EOI must be completed and submitted to the insurance company for all new or increases applied for this year. 5 Star Life will also now be the carrier for the Employer-Paid Life insurance plan.


  6. Identity Theft Plan: Employees may enroll in the ID Watchdog plan to have monitoring for all medical, criminal and financial records and resolution of any theft that may occur. Individual or Family coverage is available.


  7. There will be a NEW Dental plan with CIGNA effective September 1, 2011. This plan continues to offer the freedom of choice to see any dentist but you will save money by seeing an In-Network provider. The premiums will actually be less than the current dental premiums.   8. The NEW Vision carrier as of September 1 will be Block Vision and the premiums will be lower than the current plan rates. See the plan summary under the Benefits and Forms link for additional information.
